HOW YOU'LL MAKE AN IMPACT:
Job Summary:
- Coordinates and delivers governmental forms to patients.
- Assists with and arranges transportation for patients.
- Assists with other duties as needed
Reports To: Manager/Case Management (JC:027201)

WHAT YOU’LL NEED TO APPLY:
Required Education:
- High School Diploma or Equivalent
Required Experience:
- Two (2) years job related experience; Working knowledge of computer and Microsoft Office
