Program: Adult Access Program
­Position:  Adult Access Hub Case Manager

Description: 
The Adult Access Hub case manager will collaborate closely with the Emergency Department, WCMHS programs, as well as other public and private human service providers within the Washington County community.  The case manager is responsible for providing supportive counseling as well as case management services to persons with mental illness in urgent need. These services will be in collaboration with the Access Hub Clinician and auxiliary programs, psychiatrists, other agencies, and the community. The case manager is expected to serve between 25 to 35 clients on their caseload with varying levels of engagement. This is a regular full-time position with benefits. Must be willing to work until 7 pm 1-2 days per week.  Rest of schedule can be flexible.  EOE.

Responsibilities include but are not limited to:
  • Provide urgent face-to-face, telemedicine/phone follow up to individuals who have presented in or diverted from the Emergency Department with a mental health and/or substance use presenting issue.
  • Provide case management in collaboration with Access Hub clinician and auxiliary staff, CVMC ER, Police, WCMHS programs and other Washington County agencies serving adults.
  • Provide brief intensive case management for adults.
  • Coordinate referrals to longer term service providers.
  • Provide assessment to determine recommended treatment and referrals to appropriate providers.

The successful candidate will have the following skills:
  • Have experience working in crisis intervention.
  • Have experience in home/field-based services with an emphasis on mobile outreach.
  • Possess excellent case management skills.
  • Excellent verbal and written communication skills.
  • Create and participate in treatment team meetings and treatment coordination.
  • Demonstrate clinical interviewing and assessment skills, both oral and written.
  • Be reliable, on time, flexible and able to work both as a team member and independently.
  • Possess excellent organizational and follow-up skills.
  • Timely record keeping in electronic health records.


Qualifications: 
  • Bachelor’s Degree required with relevant experience. 
  • Experience working with adults in home/field-based settings preferred. 
  • Able to move from sitting to standing position with or without accommodation.
  • Able to drive throughout community settings with or without accommodation.  
  • Able to communicate effectively with English as the primary language.
  • Must have regular access to safe insured vehicle and clean driving record.
