Case Manager - FAST Program

Status: Full time – 32-40 hours per week

Reports to: Director of FAST

Location: 434 Jamaicaway, Jamaica Plain, MA 02130/Hybrid

Job Summary: Join us in our mission to empower unaccompanied, homeless youth through our Freedom to Achieve Stable Tenancy (FAST) program. The FAST Case Manager will oversee housing search activities, providing comprehensive support to help youth obtain housing placement and ensure long-term tenancy.

Key Responsibilities

  • Assist Case Manager with re-certifications for program participants, verifying family composition, calculating rent shares and processing leasing agreements and housing contracts
  • Apply State/HUD/DHCD regulations, policies, and procedures, educating program participants
  • Initiate and process terminations of program participation for families that have violated program requirements
  • Maintain knowledge of housing inspection requirements and procedures
  • Establish and nurture open communications with property owner/managers and tenants
  • Monitor leasing agreements and housing contracts, resolving problems and minor disputes concerning program regulations, rent shares, owner obligations and family responsibilities
  • Provide timely information required for HUD/DHCD reports
  • Collaborate with team members and other agency staff to provide coordinated and responsive service to clients, utilizing appropriate external resources
  • Perform other duties as assigned

Qualifications

  • Required: 2-3 years of case management experience with homeless individuals and families, navigation of legal, financial, and behavioral health services for housing stabilization, knowledge of service agencies and resources for homeless youth and young adults. Sensitivity and understanding of diverse populations
  • Preferred: Candidates with lived experience and reflective of the community served

ESAC has 15 staff who operate the following programs: Senior Home Repair/Falls Prevention, GED Plus, Mortgage Assistance, Benefits Enrollment, Housing Stabilization. ESAC serves 1,200 clients per year in Boston and surrounding communities.

Job Type: Part-time

Pay: $21.00 - $25.00 per hour
