A Care Management Agency is looking for Bilingual Care Managers to join our growing team! Care Managers provide services to children who meet the requirements of NYS Children’s Health Home program. They work with children, families, and service providers to create a comprehensive Plan of Health Care, identify additional service providers as necessary, and coordinate the continuing involvement of families and service providers in the execution of the plan to help them live a better quality of life.

Full Time, Part Time (minimum 20 hours each week)

Responsibilities:

• Participating in initial and ongoing training as necessary to maintain and enhance care management skills.
• Providing care coordination services for children, including meeting the client once a month in person.
• Identifying the needs of each child and coordinating services relevant to the Plan of Care.
• Documenting all case activity.
• Coming into the Brooklyn office at least once a week.

Qualifications:

• BA with 2 years of relevant experience or MA with 1 year of experience.
• Bilingual in Spanish is required.
• Solid writing and verbal communication skills.
• A valid NYS ID or NYS driver’s license is required.

Job Types: Part-time, Full-time

Pay: $22.00 - $25.00 per hour
