The Opportunity:

Functions as a Care Gap Coordinator in facilitating and prioritizing appointments required to close gaps in care. Collaborates with provider’s front and back-office staff. Makes outbound calls to members and providers to encourage, facilitate and coordinate obtaining appointments necessary to close care gaps. Documents and communicates with members and providers on referral status and appointments.

Primary Duties and Responsibilities:

  • Proactively research, locate and provide outreach to members that that have outdated contact information.
  • Uses available tools and applications to identify, list, track, and prioritize outreach activities using medical/pharmacy utilization, risk scores and gaps in care lists.
  • Establish relationship and communication with Health Coordinators, Transitions of Care Coordinators, and Community Healthcare Clinics to locate hard to reach members or difficult to find specialists.
  • Build internal and external relationships, foster trust, and respect.
  • Advocate for the AlohaCare member with key contacts with AlohaCare staff on an on-going basis.
  • Identify care gaps and outreach to members to scheduled appointments with providers to resolve on an ongoing basis.
  • Three-way calling with members and providers to accomplish scheduling of care gaps.
  • Schedule or coordinate transportation as needed for member to reach and return from care gap appointments.
  • Follow up with members after appointments to ensure tests, procedures or assessments were complete, reschedule as needed.
  • Develop subject matter expertise/intimate knowledge of specific communities; people, providers, social service agencies, faith-based organizations, schools, community centers, etc.
  • Develop key contacts and relationships with community partners, social service agencies, faith-based organizations, schools, community centers, etc.
  • Collaborate and work with internal AlohaCare departments to assist in member/patient engagement.
  • Perform other duties that support Member and Provider Engagement activities.
  • Document, track, monitor and report all member engagement and care gap closures.
  • Perform other duties as assigned.
  • Responsible to maintain AlohaCare’s confidential information in accordance with AlohaCare policies, and state and federal laws, rules and regulations regarding confidentiality. Employees have access to AlohaCare data based on the data classification assigned to this job title.

Requirements:

  • High School diploma or equivalent required
  • Requires minimum of one-year experience in Customer Service/Social Service and/or Outreach experience.
  • Must possess extensive knowledge of designated community/communities.
  • Strong verbal and written communication skills in English.
  • Diligent, detail oriented, and organized in conducting outreach activities.
  • Ability to establish trust with members.
  • Requires operation of general office equipment to include; PC, fax/copy machine and ACD Mitel Phones.
  • Intermediate skill using Microsoft Programs; Word, Excel, Outlook and PowerPoint.
  • Use of internet research tools to identify an individual’s current contact information.

Preferred Qualifications:

  • Fluent in other languages desired/preferred
  • Medical Assistant certificate desired/preferred

Mental, Physical and Environmental Demands:

  • Sedentary Work: Exerting up to 20 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects. Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time.
  • Both Inside and outside working conditions. Regular visits to state agencies, community agencies, hospitals and medical clinics
  • No environmental hazards

Salary Range: $19.75 - $26.00 an hour
