POSITION SUMMARY: The Care Manager is an integral member of the interdisciplinary team involved with each individual's care at CPC. The Care Manager coordinates services and establishes goals in collaboration with clients/consumers and their support network to promote wellness, recovery, and productive lives. The Care Manager completes clinical documentation, utilize evidence-based practices, provide crisis intervention and link individuals to community support. The Care Manager collaborates with clients/consumers, their support network, the treatment team, and other community providers to ensure positive outcomes to facilitate success at home, in school, at work and in the community.

Salary Range: $48,000

LICENSE/CERTIFICATIONS:

• Valid NJ Driver's License

MINIMAL QUALIFICATIONS:

• BA Degree in Social Science or other health related field.
• One year of experience with population served by program.

PREFERRED QUALIFICATIONS:

• BA Degree in Social Science or other health related field.
• Two years of experience working with population served by program.
• CADC preferred.
