Job Description

GENERAL SUMMARY:
Responsible for supporting the hospital based Care Transitions team to promote the continuum of care and optimal outcomes for the patient populations we serve. Works collaboratively with all interdisciplinary team members and community resources.

PRINCIPAL JOB FUNCTIONS:
1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values.

2. *Assists with post-acute referral processes and communications, including appointments and transportation needs.

3. *Answers multiple and/or designated telephone lines, priorities and routes calls.

4. *Prepares assigned departmental reports as needed.

5. *Assists Care Transitions Manager with staff scheduling process.

6. *Escalates issues to the Team Lead in a timely manner.

7. Coordinates the medication and other transition needs funds usage as requested.

8. Runs daily discharge disposition reports and makes corrections as appropriate based on the final discharge disposition.

9. *Administers, processes and documents required forms such as Important Message, PASSR’s, travel screening, referrals, ID screen forms and others as deemed appropriate.

10. *Maintains records accurately, completely, timely and confidentially according to procedures. Documents provided services.

11. *Interacts in an interdisciplinary manner and serves as a resource regarding care coordination initiatives.

12. Attends staff meetings, mandatory in-services and hospital committee meetings as required.

13. Assists in the identification and reporting of quality issues, contributes to the performance improvement plan and supports quality initiatives.

14. Participates in and maintains competencies required for the position and commits to self-directed professional growth, e.g., computer training, continuing education.

15. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

16. Participates in meetings, committees and department projects as assigned.

17. Performs other related projects and duties as assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
1. Knowledge of computer hardware equipment and software applications relevant to work functions.

3. Ability to work independently with minimal supervision.

4. Ability to modify work assignments based on requirements.

5. Ability to meet deadlines in a sometimes rapidly changing environment.

8. Ability to maintain accurate documentation.

11. Ability to communicate and cooperate with all levels of personnel, medical staff and auxiliary and ancillary departments fostering and promoting intro and inter departmental relationships.

12. Ability to maintain regular and punctual attendance.

EDUCATION AND EXPERIENCE:
High School diploma or equivalency required. Licensure or certification in a field of medical or allied health area of study preferred. Minimum two (2) years healthcare or equivalent experience preferred.
