Job Description

Responsibilities: Under the supervision of the Mason Rural Outreach/Mason Wellness Coordinator of Adult Mental Health Services, serves as a professional performing a wide range of direct and indirect mental health services to consumers with severe and persistent mental illness. Serves as an integral part of an interdisciplinary team. Services may include linking or referring to community resources for housing, employment, primary care, MDHHS/SSA benefits, rental assistance/utilities, transportation services, monitoring court order compliance, and other case management tasks. May participate as a facilitator for groups, provide crisis intervention, and facilitate hospital discharges. Responsible for detailed and timely case documentation, assessments, and person centered planning. Responsible for carrying out all activities of the program/sub-unit in such a manner that fulfills CMHA-CEI’s mission, policies and procedures.

Requirements: A bachelor's degree in social work, psychology, or any other related field is required. A minimum of one year of professional experience with adults with primary diagnoses of a serious and persistent mental illness and/or co-occurring disorder is required. On a case-by-case basis, may assess and consider an applicant with two years of paraprofessional work experience with adults with primary diagnoses of a serious and persistent mental illness and/or co-occurring disorder. Must be able to operate a computer and have the ability to gain proficiency in CMHA-CEI’s computer systems. Ability to communicate effectively both in writing and verbally is required. Possession of a valid Michigan driver’s license and access to reliable transportation for job related use is required.

Conditional Employment Requirements: Employment offers for this position will be contingent upon the results of a background verification and credentialing procedure consisting of: a Michigan State Police clearance check, verification of a professional license, Medicaid/Medicare verification, verification of an acceptable Michigan driving record, educational background check and verification of an acceptable work history. Must be able to pass a pre-employment drug screen.

Salary/Hours: $23.34 - $26.54 per hour. 40 hours per week. 8-5 M-F, occasional evening, weekend, or holidays may be required.
