Job Description

ESSENTIAL FUNCTIONS: The FSCC/Wraparound Facilitator is responsible for all aspects of the Wraparound process. They work with families with children at risk for abuse and neglect, at risk of psychiatric hospitalization, with Serious Emotional Disturbance, as well as with youth with or at risk for juvenile justice/wayward involvement and/or exiting the RI Training School. FSCCs work with families, youth, their natural supports, DCYF involved staff, and other community providers to implement a wide range of FCCP Wraparound services including initial/on-going crisis stabilization, ensuring effective linkages to needed support and/or treatment services, and longer-term Wraparound Family Team plan development and meeting facilitation for families who need and agree to participate in full Wraparound services.


SPECIFIC DUTIES/RESPONSIBILITIES:

Outreach and Family Engagement:

  • Initiate intensive outreach to assigned families within 1 business day of assignment for routine referrals; within 4 hours for urgent DCYF referrals/meet them within 24 hours; and meet face to face within 2 hours for emergency DCYF referrals.
  • Schedule initial Intake visits with assigned families within 5 business days (except earlier as above for urgent or emergency referrals)
  • Maintain at minimum twice weekly contact with assigned families, at least once per week in person contact, throughout the families engagement with FCCP. Partner with other FCCP staff including Supervisors and FSPs to ensure adequate services are provided.
  • Implement intensive outreach and re-engagement strategies for any family who cancels or no shows weekly appointments; including consulting with Supervisor and notifying DCYF whenever appropriate and in accordance with the contractual requirements.

Family Assessments:

  • With supervision, conduct bio-psycho-social and strength-based, culturally competent family needs assessments.
  • Identify emergency or urgent needs according to protocols including safety, risk and caregiver protective capacity.
  • Provide crisis stabilization services during initial 30 days as well as whenever needed throughout families involvement; includes developing and monitoring implementation of Safety Plans with families to address identified safety concerns
  • Review legal and ethical issues including mandated reporting laws and clients rights.

Wraparound Facilitation:

  • Facilitate the Wraparound process including family engagement, development of Care Plans, Wraparound Team development, and supporting self-efficacy through the development of natural supports.
  • Connect families to culturally relevant community based resources and supports.
  • Schedule and facilitate Family Care Plan and Wrap Team meetings.
  • Transport clients and family as needed.

Documentation Management:

  • Obtain or ensure collection of required records (e.g. evaluations, assessments).
  • Maintain required program documentation and individual child/family records.
  • Obtain HIPAA consent and other required releases, including privacy notices.
  • Assure the procurement of primary child diagnoses when appropriate.
  • With the family, develop and revise the family care plan.
  • Complete data collection as required by the Rhode Island Family Information System.

Advocacy:

  • Advocate for families during the family care planning.
  • Assist families in accessing parent advocates or peer mentors.


QUALIFICATIONS:
Requires minimum of Bachelor degree with 2 years of experience, or a masters degree with 1 year of experience working with children and families with complex needs, at risk for involvement in the child welfare system. Candidates should be familiar with the Rhode Island system of care and have an understanding of how to engage families, community partners, and natural supports in a collaborative Wraparound process. Candidates should be available for a flexible schedule including evening or weekend meetings as is needed by families.


FSCCs are required to become certified in the Wraparound process within one year of employment. Training for certification is provided by the program.


PHYSICAL REQUIREMENTS:
This position requires residential and community visits, employees in this position must have the ability to:

  • Travel to and from clients residence, community locations and office site, which could include using walkways, stairs and/or elevators.
  • Ability to lift up to 20lbs.
  • Ability to communicate by voice, use sight and hearing.
