Job Description

Position Summary:

  • The Family Support Worker provides support and assistance to Case Managers on their work to facilitate timely permanency for children and families in need of care.

Location:

  • Topeka, KS

Bonus:

Education:

  • A GED or High School Diploma

Experience:

  • Experience in working with vulnerable and at-risk children and older youth required
  • Intermediate computer skills required

Licensure/Certification:

  • Valid drivers license
  • Valid Auto Insurance
  • At least 21 years old

Essential Duties:

  • Driving is required as FSW’s provide transportation and visitation support as needed for our clients; use of fleet vehicles is available
  • Supervise parent/child and sibling visits, assess progress in family functioning
  • Present as a positive role model for clients and families
  • Assist clients in finding community resources (housing, utilities, mental health, and health care)
  • Teach parents daily living skills such as budgeting, housekeeping, and parenting techniques
  • Provide in-home support to children and families
  • Complete and submit case activity logs ensuring all logs are entered in the month in which the activity with the client occurred as required per our DCF contract
  • Participate in case management meetings
  • Participate in case planning conferences as needed
  • Case plan review, correction, and processing including forwarding to DCF and court as required
  • Attend court hearings as required; present professionally and communicate effectively
  • Review daily, weekly, and monthly reports for accuracy and follow-up on missing or incorrect data as necessary
  • Monitor, update, and correct CMS and ensure all case information remains current and accurate
  • Assist in preparation of file and reconciliation for DCF audits as required
  • Perform a variety of duties to ensure smooth work flow and to meet contract outcomes
  • Attend all required trainings
