*Job Overview*

We are seeking a dedicated Home Care Nurse to join our team to do in take assessments for new clients. We pay a flat rate for each assesssment and an asessment typically last an hour.

This role will be on a as needed basis.

*Responsibilities*

- Monitor patient vital signs and condition, reporting any changes to the healthcare team

- Educate patients and their families on proper home care procedures

- Collaborate with other healthcare professionals to create individualized care plans

- Maintain accurate patient medical records

-Answer questions caregivers may have

-Assess clients as needed

*Skills*

- Knowledge of infection control protocols

- Experience in phlebotomy and mammography is a plus

- Strong understanding of nursing home procedures and patient care

- Excellent communication and interpersonal skills

Experience:

• Nursing: 1 year (Preferred)

License/Certification:

• BLS Certification (Preferred)
• RN (Preferred)

Ability to Relocate:
