Astria Health is seeking a Home Health Aide to join our team! This position is for Home Health.

As a Home Health Aide, you will be responsible for the direct personal patient care in a patient's home according to a patient's specific plan of care prepared by the RN Case Manager. You will:

• Provides personal caregiving services in accordance with an established plan of direct care
• Provides for the personal needs and comfort of people in their own homes

• Assists patients with activities of daily living; including bathing, dressing, grooming and toileting

• Performs general housekeeping activities
• Provides transportation assistance to and from activities and appointments
• Offers compassionate companionship to patients

• Monitoring and reporting change of condition of patient
• Collaborate effectively across organizational boundaries.
• Adheres to the core values and standards of the organization.
