As a home health / hospice nurse, you may be responsible for:
Skilled Nursing and Wound Care skills!
Assessing the patient’s physical, psychological, and social needs
Developing an individualized care plan for each patient and providing care that is in accordance with the plan
Teaching basic self-care skills to patients or their caregivers to help them manage chronic conditions at home
Promoting a patient’s independence by providing daily care activities such as bathing, dressing, preparing meals, and cleaning the home
Monitoring a patient’s physical and emotional progress or changes throughout treatment
Meeting with family members to ensure that they have a clear understanding of the plan of care for their loved one
Assisting with exercise programs and other therapies (physiotherapy, speech therapy, etc.)
Assessing vital signs and taking blood samples to monitor the patient’s health
Performing administrative duties such as updating physicians on patient progress
