Job Description

Summary

The role of the Social Worker is to assess and assist the patient and family’s ability to cope with terminal illness.

Essential Duties & Responsibilities

  • Initiates appropriate preventative social work procedures to provide total patient care.
  • Assess the patient and family’s ability to cope with the terminal illness.
  • Provide crisis intervention to resolve psychological problems placing patients and families at high risk.
  • Assist patient/family in utilizing available community resources and formulating advance directives.
  • Assist patient and family in long range planning and decision making including alternative living placement.
  • Evaluate the continuing needs of patients and families and their ability to cope with the terminal illness.
  • Provide information to agency regarding changes in status of patient.
  • Maintain necessary records and forms for analysis and planning of service for establishment of priorities for care.
  • Responds appropriately in urgent and/or emergency situations.
  • Evaluates and notes the individual patient response to care defined in the social work care plan and medical records.
  • Supports standards of care through adherence to existing policies and procedures.
  • Actively participate in interdisciplinary team conferences
  • Participate in QAPI Program
  • Other duties as assigned

Qualifications

Education/Experience

Bachelor’s or Master’s degree in Social Work from an accredited school. Two (2) years of social work experience in a healthcare setting.

Language Skills

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from managers, patients, and the general public.

Reasoning Ability

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Computer Skills

To perform this job successfully, an individual should have knowledge of Electronic Medical Records software and Business communication software.

Certificates and Licenses

Master of Social Work (MSW) from a school of social work accredited by the Council on Social Work Education; or baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in psychology, sociology, or other field related to social work and is supervised by an MSW.

Job Type: Full-time

Pay: $60,000.00 - $72,800.00 per year
