Lutheran Hospice is currently recruiting for a full-time Hospice LPN.

We are seeking someone who is a compassionate, dedicated, and caring professional to join our team!

Responsibilities:

  • Providing direct care to patients and caregivers under the supervision of a registered nurse.
  • Will follow the plan of care, providing treatments, and working collaboratively with the members of the team to provide excellence in hospice care in accordance with established policies, procedures and standards.
  • Strong accountability to the Director of Clinical Services.
  • Manage patient needs and responds to caregiver concerns as needed, to ensure a compassionate approach to comfort and care
  • Completes an initial assessment of patient and family to determine hospice needs
  • Provides professional nursing care by utilizing all elements of nursing process
  • Uses health assessment data to determine nursing diagnosis
  • Initiates appropriate preventive and rehabilitative nursing procedures
  • Administers medications and treatments as prescribed by the physician in the physician’s plan of care
  • Counsels the patient and family in meeting nursing and related needs
  • Provides health care instructions to the patient as appropriate per assessment and plan
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care
  • Provides and maintains a safe environment for the patient
  • Assists the patient and family/caregiver and other team members in providing continuity of care
  • Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver

Qualifications:

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Two years nursing experience. Facility/hospice or medical/surgical/oncology experience is preferred.
  • Currently licensed as an LPN/LVN in the State. Current CPR certification. Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication, and organization skills. Must demonstrate effective communication skills and be able to work well with others including team members, patients, caregivers, referral sources, physicians and their staff, facility staff members and the community.
  • Ability to manage time, resources and paperwork requirements while meeting the needs of patients and caregivers. Must work well under pressure and during crisis situations.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Must be able to travel throughout the service area.
  • Must be flexible to assist the hospice program in providing 24 hours, 7 days per week care. Some evening, night, weekend, and holiday work will be necessary.
  • Must demonstrate professional and personal skills sufficient to properly deal with difficult issues and high emotional responses related to death and dying issues. Must understand philosophy of hospice concept and the needs of the terminally ill.
  • Demonstrates ability to effectively learn and use computer and required software programs to ensure a true and correct electronic medical record for each patient.
  • Must be able to utilize technology appropriately as needed to meet the requirements of the position
  • Detailed oriented and strong problem solving skills
  • Excellent verbal and written communication skills
