Job Description

Join our growing team! We are looking for a full-time dedicated and experienced LPN with strong initiative needed to help with coordinating patient referrals for our hospice program. The intake nurse evaluates patients/clients referred to the agency, identifies insurance coverage, plans, and coordinates referrals for hospice services through coordination with field staff.

RESPONSIBILITIES

  • Evaluates provided information of patients referred for hospice services.
  • Identifies insurance coverage of hospice services.
  • Collaborates with the hospitals’ case management to facilitate safe, early discharge.
  • Consults with physicians, nurses, social workers, discharge planners, and other disciplines to establish a coordinated home plan of care.
  • Ensures that all intake forms are complete, clear, and within scope of service.
  • Communicates with field staff and other hospice departments regarding status of referrals.
  • Orders DME and specialty supplies for referrals.
  • Performs other duties as requested/assigned.

QUALIFICATIONS:

  • Required graduate of an accredited school of nursing
  • Required current license to practice as a licensed practical nurse in Louisiana
  • Required License must be unencumbered
  • Minimum of two years experience, at least one of which is in the area of public health, home care, or hospice nursing preferred.

SKILLS:

  • Demonstrates an ability to work with other health care clinicians.
  • Demonstrates ability to handle stress on the job.
  • Ability to read and communicate effectively in English
  • Excellent interpersonal, telephone, documentation, and communication skills.
  • Excellent computer skills (i.e., Word, Excel, Outlook, Power Point, etc.)
  • General business experience
