The Manager of Complex Case Management has primary responsibility for professional oversight and supervision of designated staff across system. Provides for an efficient and coordinated approach to complex case management, transition of patients to aftercare providers, referral to community resources and readmission interventions. Serves as Manager,

REQUIRED:
1. Graduate of an accredited school of nursing, OR; Masters of Social Work (MSW) OR; B.S. degree in related field with strong clinical background.

2. At least five (5) years previous management experience.

3. Five (5) years previous experience in case management, or social services.

4. Ability to interact with patient, families, physicians and hospital staff (including coworker/peers).

5. Excellent oral and written communications skills. 6. Experience in CQI concepts and tools and techniques.

PREFERRED:
1. Licensed Clinical Social Worker (LCSW)

2. Seven (7) years' experience in management, social services or related field.

3. Certified as Certified Case Manager.
