Job Description

We are a Home Hospice provider seeking an MSW to join our growing team. We provide care to our patients where they live, be that in private homes or assisted living facilities in Columbus. We are primarily looking for a compassionate and resourceful social worker to help support our patients and families at end-of-life and throughout the bereavement period. We occasionally have social work needs on home health side of the business as well - typically for assistance with resource identification and enrollment assistance.

Qualifications and Skills:

  • Master of Social Work (MSW) (Required)
  • Ohio LSW or LISW (Required)
  • Strong clinical skills (Prior Home Health/Hospice Experience Preferred)
  • Minimum requirement of 1 year medical social work experience
  • Knowledge of Federal and State laws and guidelines pertaining to Home Health and Hospice
  • Demonstrates good communication and public relations skills
  • Computer proficiency desired
  • Knowledge of Home Care and Hospice Medicare/CoP regulations

Summary:

Work with a multi-disciplinary team of clinicians providing evaluation, treatment planning and implementation, and support for patients in end-of-life situations. Social Worker will follow their client and family from evaluation to death or discharge, and will follow some family members following death to provide bereavement support.

Responsibilities:

  • Evaluates patients and their caregivers by conducting a thorough psychosocial assessment related to the patient's illness and environment.
  • Develops reasonable, specific and measurable goals that support the plan of care, as agreed upon by the patient/caregiver.
  • Works with interdisciplinary team to ensure all physical and psychosocial needs are met in a timely fashion.
  • Develops, evaluates, and revises patient care goals in conjunction with the patient and family.
  • Coordinates goal-directed, cost-effective care for primary patients and demonstrates appropriate use of community resources to deliver comprehensive individualized care.
  • Communicates with MD, family, and interdisciplinary care providers to clarify care and increase awareness of significant changes in condition in a timely manner.
  • Maintains effective working relationships with patients/families, referral sources, physicians, community resources and insurance representatives.
  • Works with hospice administrator to plan and carry out bereavement support activities; assesses for complicated grieving and makes appropriate referrals.

You will have a flexible schedule and utilize an electronic EMR which will help limit travel time and visits to the office. No on-call requirement!

Pay: $30.00 - $40.00 per hour
