Job Summary and Qualifications

The Patient Care Coordinator is responsible for the programs and resources that support the coordination of ED patients through complex healthcare environments for the optimization of care, outcomes, and patient experience. Responsible for facilitating patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. The Patient Care Coordinator guides patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes while consistently providing exceptional customer service. Serves as a point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services. Additionally, the Patient Care Coordinator drives and fosters a culture of patient and family-centered care and service excellence while improving the patient and family experience and patient relations.

What you will do in this role:

  • In this role, you will review patient case lists generated by technology-enabled data triggers to identify patients that could benefit from intervention
  • Your role communicates with physicians (in the hospital and in the community) about patients' potential for referral / follow-up treatment
  • You will schedule follow-up appointments, as noted by the physician, and provide information and reminders to patients & physician offices
  • You will coordinate, investigate and resolve consumer grievances concerning the quality of care complaints by providing a formal grievance mechanism for the hospital
  • You will understand problem resolution processes
  • Your role directs the effort to collect, analyze, and evaluate consumer satisfaction data
  • You may assist with discharge call backs to patient
  • You will utilize feedback to facilitate systems improvements to better meet the consumer's needs
  • Your role supports the accountability scorecard process, e.g. process metrics, patient verbatims, close the loop
  • You will provide feedback to leadership through daily communication and weekly status updates in regards to target population in order to enhance care coordination and patient experience.
  • You will collaborate with care team members and suggests ideas for development.
  • You will participate in frequent patient rounding in order to coordinate, investigate and resolve consumer grievances concerning the quality of care complaints
  • Your role significantly improves specific processes; recommends and facilitates initiatives to improve stakeholder value.
  • You will demonstrate competency and compassion and performs consistently under routine conditions/situations.
  • You will apply appropriate tools and methods to guide patients through the system.
  • You will engage providers and patients in a program that will improve outcomes and the efficiency of medical care
  • When appropriate, you will update patient and families regarding plan of care, medications and treatments as needed
  • You will field non-clinical phone calls and questions from patient/family and when appropriate, provides resources with empathy and compassion
  • You will participate in creating an environment and performs job functions in a manner that exemplifies the mission, vision and values of the organization.
  • You will encourage collegiality and collaboration when integrating pilot services with other departments.
  • You will proactively assist patients with registration process as needed, offers comfort items as appropriate, and demonstrates anticipatory service to patients and families in the waiting area.
  • Your role evaluates and provides input regarding the utilization of resources to meet the needs of the target population.

What qualifications you will need:

  • Associates Degree required
  • Minimum 1 year of customer service experience or patient experience
  • Employee completes initial and ongoing training and competencies as defined by service line, facility and department/unit which are specific to the needs of the patient population served (if applicable).
