Pride Health is actively hiring a Physical Therapist (PT) to join our client's Home Health Team.
Job Summary
• We are seeking an experienced and compassionate Physical Therapist (PT) to provide high-quality home health services to patients in their homes.
• The Physical Therapist will develop and implement individualized plans of care to help patients improve mobility, strength, balance, safety, and overall functional independence.
Local Pay: $55.00 to $60.00/hour (W-2)

Schedule:
Hours: 8 hours per day / 40 hours per week
Schedule: Monday–Friday, 8:00 AM–4:30 PM
On-Call/Weekend Requirement: None
Productivity Expectation: 30 points per week
Coverage Area: Highland County, Clinton County, Fayette County.
Mileage reimbursement: Yes, between patient visits from the first patient to the last patient.
Patient Care Setting: Home Health
Dress Code: Any professional attire/scrub color.

Key Responsibilities:
• Provide skilled home health PT services, including gait/balance training, strengthening, mobility improvement, and home safety interventions based on individualized plans of care.
• Maintain accurate clinical documentation, coordinate with the care team, monitor patient progress, and meet the 30-point weekly productivity expectation.
Required Qualifications:
• Current and active Physical Therapist (PT) license in the State of Ohio.
• Minimum 1 year of Home Health Physical Therapy experience.
• Strong clinical assessment, treatment planning, and patient education skills.
• Ability to work independently and manage a patient caseload effectively.
• Excellent communication and interpersonal skills.
Preferred Qualifications: Experience with Homecare Homebase (HCHB) EMR.
Traveller Pay package:(Applicable only if your home location is more than 50 miles from Wilmington, OH 45177)Gross Pay: $64.25/Hour (Weekly: $2,570.00)Take Home pay: $54.17/Hour (Weekly: $2,166.80)Pay Structure:Stipend: $890.00/Week ($22.25/Hour)W-2 Rate: $1,680.00/Week ($42.00/Hour)
