Re-entry Health Conductor/Health Navigator

For the Formerly Incarcerated in East

Contra Costa County

Position is 40 hours a week (100% FTE)

Background

The Center for Human Development is a community-based organization that offers a spectrum of services for at-risk youth, individuals, families, and communities in the Bay Area. Since 1972 we have provided wellness programs and support aimed at empowering people and promoting positive growth.

The African American Health Conductor Program and Re-entry Services is an innovative partnership between Contra Costa Health Services and the Center for Human Development. The goal of the African American Health Conductor Program and Re-entry Services is to improve access to health care and services among formerly incarcerated living in Contra Costa County.

Job Description

The Re-entry Health Conductor is an integral part of a medical team that provides comprehensive health services, outreach, support, patient navigation, patient follow-up and health coverage assistance to formerly incarcerated men and women. The Re-entry Health Conductor functions as a bridge for formerly incarcerated to health care and public systems, that have traditionally been difficult to access due to their complexity, cultural capacity, and trust level. This position will be based in East Contra Costa County.

Principal Duties

  • Conduct targeted outreach to formerly incarcerated men and women.
  • Provide Pre-release support inside Martinez and West County Detention (12 hours per. Week)
  • Co-facilitate support group called (REMEDY) for formerly incarcerated.
  • Work closely with Health Clinic staff and medical doctors registering patients into the Transitions Clinic (a clinic specifically designed for those formerly incarcerated)
  • Educate formerly incarcerated about health care coverage options and provide enrollment assistance into health care coverage programs.
  • Maintain patient files, including case notes and confidential information via the EPIC database.
  • Make calls to patients to remind them of their medical appointments and to attend the REMEDY support group.
  • Make referrals to both county and community resources.
  • Attend meetings that include, the Re-entry Health Conductors staff meetings, RHD Joint Staff meetings, CHD All staff meetings, Parole and Community Team (PACT) meetings, including meetings held by the Re-entry Networks.

(There is a 2 -week training process for this position that will be provided)

Requirements: A qualified candidate will demonstrate the following characteristics for successful performance in this position:

· One to two years of experience working with and commitment to incarcerated individuals and other vulnerable populations and communities

· Familiarity with East Contra Costa County

· Ability to negotiate and navigate across community/geographical lines.

· Strong active listening, and verbal skills

· Computer proficiency to schedule patient visits, input data, create reports and flyers, and receive and respond to emails.

· Demonstrated ability to establish and maintain effective working relationships with the public, co-workers, supervisors, and clinic staff.

· Ability to organize and set priorities and manage multiple tasks.

· Ability to work independently, and as part of a team

· Ability to adjust time to allow for some weekend and evening events

· High School graduate or GED

· Clear TB test

· Must possess a valid California Driver’s License and proof of insurance or have access to another form of reliable transportation

· Ability to pass a Live Scan background check.

Reports directly to:

Contra Costa Health Services Program Coordinator for the African American Health Conductors, Re-entry Programs and the Transitions Clinic

Salary:

Starting salary $23.75 an hour. Benefits include medical, dental, 19 days of vacation, 12 holidays, and a retirement plan, which are all pro-rated based on FTE. This is a non-exempt position, 40 hours per week.
