JOB SUMMARY:

Coordinates pre-intakes, registration process, and case management linkage to appropriate community services for the behavioral health programs of Catholic Charities, Diocese of Trenton.

DUTIES AND RESPONSIBILITIES:

• Coordinates pre-intakes, registration process, and case management linkage to appropriate services for the behavioral health programs of Catholic Charities, Diocese of Trenton.
• Receives and screens initial requests for services offering guidance with crisis navigation to internal and external parties as appropriate.
• Assuring department coverage Monday - Friday 11am to 8pm
• Provides case management linkage for consumers to appropriate Catholic Charities services.
• Reviews psycho-social materials to determine appropriate level of care
• Knowledge of various insurances and ability to adequately explain benefit information
• Provide detailed description of agency programs
• Coordinate follow-up with referral sources
• Other duties as required
