Compass Healthcare Consulting & Placement is conducting a search for an experienced bilingual Chinese, NYS RN Care Manager for D-SNP, Dual Special Needs Plan, Case Management. Qualified candidates will have prior Medicare Advantage and/or D-SNP, Dual Special Needs Plan RN Care Management experience. Position is Fully Remote, doing telephonic Case Management. Must have NYS RN. Bilingual Chinese / Cantonese / Mandarin, languages in need!

RN Care Manager Responsibilities:

Reporting to the Supervisor of care management, this position will provide telephonic case management to our members including medical, social, psychological, physical and spiritual needs by developing, implementing and monitoring the member's care plan in conjunction with the PCP, caregivers and other team members in order to help meet the member's needs. managed care nurse, managed care rn, nursing, rn, nurse, care management

Qualifications:

• NYS RN license required

• CCM preferred

• At least 1+ years of RN experience in managed care, MLTC, Medicare Advantage, Dual Special Needs Plan or CHHA Home Care RN with 1+ years of Home Care Case Management experience.

• Bilingual Chinese (Cantonese/Mandarin), required

• Prior experience in an interdisciplinary service delivery environment preferred. Working knowledge of Medicare and Medicaid regulations

Excellent Salary based on relative experience and great Benefits! Salary Range $102,000-105,000
