SCHEDULE M-F 9a-5p (Alternating weekend coverage)

- $120,000-$135,000/Annually

Responsibilities
Lead a team of RNs who are responsible for delivering direct high acuity care to patients in home recovery care .
- Oversees daily clinical and field operations , including direct supervision and in-home shadow visits with the RN team.
- Works closely with the recovery care coordinators and leader s to facilitate communication and coordination between all members of the care team.
- Guides RNs to promote effective performance and delivery of quality in-home care services through effective onboarding, education, mentoring and/or remediating clinical skills/processes/critical thinking .
- Assists team members in establishing immediate and long-term therapeutic goals, in setting priorities and in developing the care plan.
- Ensures effective communication of discharge information to other clinical departments or members of the c are t eam.
- Participates in the market screening and interview process of new employees and makes recommendations for the employment of individuals.
- Provides the clinical orientation process through didactic, "at the elbow" and in-home shadowing methods.
- Actively participates in performance improvement activities.
- Acts as an acute care RN as needed. Performs other duties as assigned.
Qualifications
Associate or bachelor's degree in n ursing.
- Current RN license, specific to the state(s) you are assigned to work.
- Three years of adult acute inpatient medical/surgical experience within the last five years.
- One year of supervisory/management experince as an RN in a healthcare environment.
- Current CPR certification.
- Valid driver's license, reliable transportation and liability insurance (if applicable) .
Preferred
One year of RN experience in a h ome h ealth or h ospice environment.
- Adult medical critical care.
