Job Description:

The RN Transitions Coordinator—Acute Rehab manages and coordinates post-acute referral process (i.e., Home health/Acute Rehab) within assigned Centra hospitals and/or facilities. Serves as a resource, educator, and post-acute subject matter expert in assisting case management colleagues and physicians to identify patients that would benefit from, and be eligible for, post-acute services. Collaborates with physicians, clinical staff and case management to define indicators that would trigger a post-acute consult (i.e. Home health or Acute Rehab). Provides ongoing education relative to post-acute regulatory changes and interventions as they relate to specific diagnoses and disease states. The Clinical Transitions Coordinator is accountable for all aspects of post-acute care coordination in the acute care setting.

Requirements:

· Required Education: Graduate of an accredited Nursing Program

· Preferred Education: Bachelors of Science Degree in Nursing (BSN)

· Required Experience: Three (3) years of Post Acute (e.g., Home Health or Acute Rehab)

· Required Certifications and Licensures: Current Virginia or Multistate Nursing Registration. Hold a current, active American Heart Association Basic Life Support (AHA BLS) course completion card.

Effective April 1, 2024, candidates seeking employment in this role at Centra must possess a valid and current American Heart Association Basic Life Support (AHA BLS Provider or AHA BLS Instructor) course completion card. Proof of certification will be mandatory prior to commencing employment. Both AHA and RQI cards are acceptable forms of certification.
