Key Details:

  • Title: School-Based Health Center Patient Coordinator
  • Pay: $19.60 - $22.00/hour, depending on experience
  • Bilingual Pay Policy: Eligible for a $1.25 per hour increase after passing a language proficiency test.
  • Schedule: Full-time, availability required between 8am-6:30pm, Monday-Friday.
  • Location: 1133 N Main Ave, Gresham, OR.
  • Note: When the School-Based Health Center is closed, you’ll work at Wallace’s main clinic center.

Who You Are:

  • Fluent in both Spanish and English.
  • A customer service expert, especially in a dental or medical setting.
  • Thrives in a fast-paced environment.
  • Excellent communicator, both in person and over the phone.
  • Detail-oriented, organized, and skilled at juggling priorities.
  • Proficient with Microsoft Office Suite.
  • Experience working with clients facing barriers such as poverty, language, mental illness, or housing.
  • Able to pass a pre-employment background check.

Bonus Points For:

  • 1+ year of managing providers’ schedules in a dental or primary care clinic.
  • EMR system experience, especially with Epic.\
  • Knowledgeable in medical records, particularly HIPAA.
  • Skilled in de-escalation techniques.

What You’ll Do: This role is all about bringing healthcare directly to students in a way that’s approachable, supportive, and tailored to their needs. You’ll be the welcoming face of our school-based clinic, creating a safe environment while supporting operations at Wallace Medical Clinic. We’re looking for someone ready to engage with the community, collaborate with our team, and deliver outstanding patient care where it’s needed most.


Your key responsibilities:

  • Welcome and introduce students to Wallace services, addressing any questions.
  • Provide compassionate care during patient check-ins for medical and dental appointments in the EMR system.
  • Confirm appointments and collect/verify patient insurance and demographic data.
  • Record, scan, and maintain patient data in electronic health records while identifying those who may need insurance assistance.
  • Collaborate with dental, medical, and administrative staff to ensure timely, safe patient care.
  • Adhere to HIPAA regulations and ensure patient confidentiality.
  • Perform office tasks such as photocopying, typing, phone calls, and more as directed.
  • Coordinate referrals, specialty appointments, and assist with scheduling follow-ups and transportation as needed.
  • Assess and address social determinants of health such as housing and food access to close care gaps.
  • Maintain accurate documentation of care coordination services and ensure timely entry into the EMR.
