Job Description

AVCMHA provides State funded public mental health services to Iosco, Ogemaw, and Oscoda counties in northern Michigan. Services are provided to people with an Intellectual or Developmental Disability (I/DD), a Serious Persistent Mental Illness (SPMI), and/or those who may be Seriously Emotionally Disturbed (SED).

Candidates must be able to work a flexible schedule and travel throughout the Board's coverage area and, at times, around different areas of the state. Possession of a driver's license valid in the State of Michigan, vehicle insurance as required by Michigan Law, and a good driving record; be a U.S. Citizen or have possession of necessary papers allowing holder to work legally within the United States.

Education and/or Experience:

  • High School or Associate degree
  • One year experience in the mental health field

Works with individuals served receiving B3 services and/or specific waiver services to assure all necessary supports and services are provided to enable the individual to achieve community inclusion and participation, productivity, and independence in home and community based settings. Responsible for advocacy, coordination, and linkage of services to provide continued care and to avoid rigidity, fragmentation, misuse and/or inaccessibility of services. Must possess the ability to link the individuals served to services and coordinate the various services to achieve successful outcomes. Provides services to assist the individuals served in gaining access to needed medical, social, education, therapeutic, and other support services. Major functions and core elements of the role includes service plan development, linking and coordination of services, reassessment/follow-up and monitoring of services and advocacy.
