Job Description

KVC is seeking a Case Manager!!!**This position will cover Hiawatha and Marshall County**

There is a $2000 Sign on Bonus associated with this position. It only applies to External Candidates

Position Summary:

  • The Case Manager provides case management services to children and families in order to facilitate safety, permanency, and well-being.

Location:

  • Hiawatha, KS

Education:

  • A bachelor’s or master’s degree in social work, community counseling, human development, child and family development, applied family and youth services, public health, health sciences, trauma studies, sociology/social services, substance abuse/addictions, early childhood education, or psychology from an accredited college/university.

Experience:

  • Previous experience with children and family services preferred.

Licensure/Certification:

  • Valid driver's license
  • Valid Auto Insurance
  • At least 21 years old
  • Licensure by the Kansas BSRB is preferred.

Essential Duties:

  • Actively seek to understand and support families’ lived experience and individual needs.
  • Needs include, but are not limited to, developmental stages, trauma, poverty, gender, race, ethnicity and language.
  • Build and maintain strong engagement with clients, placement providers, DCF, coworkers and other community partners.
  • Actively share relevant and appropriate information with family and involved professionals.
  • Provide timely responses to all requests.
  • Provide home and community-based services to clients.
  • Services include, but are not limited to, observation of caregiver/child interaction, maintain both existing and
  • developing connections, resource coordination, on-call crisis intervention, safety planning, skill building, transport clients and conduct drug-screens to ensure safety and well-being.
  • Utilize information gathered through assessment process to advocate for the client’s best interest.
  • Advocacy includes, but is not limited to, provide court testimony, attend school meetings and coordinate mental health services
  • Complete required documentation in a timely and thorough manner.
  • Documentation includes, but is not limited to, case logs, comprehensive assessments, court reports and social histories.
