THEMA Health Service (Hospice) is looking for an extraordinary social worker (Must have a Masters of Social work degree or LCSW) with a hospice heart who wants to work for a company that cares. We have a PRN position available that has the potential to be full time (if desired) as we grow. We have an amazing team and are looking for quality people to help us provide a exceptional care. We are dedicated to providing compassionate hospice care to those in need.

Previous hospice experience is required.

We would ideally like to hire a MSW or LCSW that lives in Green Valley OR is comfortable with the commute from Tucson to Green Valley as the job is primarily in Green Valley.

Must be willing to travel to wherever the patient calls home!

This starts as a PRN position for Green Valley and surrounding areas. PRN staff actually get paid per visit, not by hour, so the rate listed is just approximate. But full time would be a 40 hour a week position.

Duties:

- Conduct assessments and evaluations of individuals to determine their needs and develop appropriate treatment plans

- Collaborate with other professionals, such as Nurses, physicians, Chaplains and educators, to develop comprehensive treatment plans

- Provide counseling and support to individuals and families during end of life situations.

-Assist individuals in accessing community resources and support services
- Maintain accurate and up-to-date documentation of client interactions and progress
