Job Description

Summary:


Provide diagnosis management (treatment education patient empowerment) and case coordination for patients with spine-related disorders including neck and back pain radiculopathy spinal stenosis and related neuromusculoskeletal problems.

Collaborate in the development implementation and direction of the Low Back Pain Pathway under the Musculoskeletal Health Program

Provide education and support for Coastal Medical clinicians and staff to optimize musculoskeletal spine care of patients.

Serve as a resource for primary care clinicians by sharing best practices and developing efficient workflows through the Low Back Pain Pathway and other initiatives as they develop.

Participate in the data collection analysis dissemination and publication as needed.


Responsibilities:


Provide or coordinate complete primary spine care according to established standards of care and principles of evidence-based medicine.

Conduct comprehensive history taking examination diagnosis and treatment based on principles of shared decision making.

Educate patients and their caregivers on wellness prevention and disease management by providing materials and resources to patients and families or directing them to appropriate sources of information.

Determine the need for consultation with other healthcare professionals for the comprehensive management of patients and refer as appropriate to identified high-quality cost-effective specialists who provide reliable patient service.

Order laboratory imaging studies and ancillary services as required in the diagnosis management and monitoring of care including timely response to results and ensuring communication of results to patients or their caregivers.

Collaborate with interdisciplinary teams in closing gaps of care clinical quality improvement and variation reduction to ensure success in population health management efforts and success in value-based care contracts.

Participate in care conferences and daily huddles according to the standards established in the office to ensure optimal coordination of care for complex patients and the functioning of team-based care.

Consult with the Musculoskeletal Health Medical Director and Coastal Management as needed on issues related to patient care patient experience the resolution of patient complaints and service recovery.

Provide clinical oversight of non-physician employees including advanced practitioners and medical assistants.

Document in the electronic medical record all care rendered according to Coastal�s documentation standards including note completion locking notes and reviewing clinical documents.

Maintain professionalism in the documentation of patient concerns and communication in the medical record with colleagues and clinical team members involved in the coordination of care.

Maintain confidentiality of all patient information according to federal and state regulations and guidelines.

Demonstrate shared responsibility for patients by providing clinical care in accordance with relevant policies and standards of care.

Education and Experience

Doctor of Chiropractic degree from an accredited chiropractic college.

National Board of Chiropractic Examiners (NBCE) Board Certified or Board Eligible

RI Chiropractic Medical Licensure.

Continuing Medical Education (CME) as required by the State Board of Licensure and designated specialty board if applicable.

Able to maintain confidentiality with all aspects of information (including patient data) in accordance with Coastal Medical�s philosophy and policy as well as state and federal regulations.

Must handle the most sensitive and confidential matters with the utmost discretion.

Physical Effort and Dexterity

Sit and stand for prolonged periods of time.

Work and move among all departments of Coastal Medical group.

Excellent hand/eye coordination.

Lift up to 15 pounds.

Manual dexterity to operate personal computers and standard office machines and equipment as it pertains to one�s position.

Visual Acuity Hearing and Speaking

To transfer information from paper to computer from computer to computer and to communicate in person and on the telephone.
