Description of Job

This is a temporary part-time, non-exempt position. Shiftwork is tied to this position including day, night, evening, weekends, and holidays.
Anticipated Begin Date: 5/13/2024 thru 01/31/2025
Please Note: anticipated start and end dates may change depending on recruitment status.

This position exists to provide direct hands-on care to facility residents including assistance with activities of daily living, documentation of care provided, and awareness of psychosocial and physical needs/safety of residents, all performed in a manner consistent with the standards of the profession, and legal and regulatory requirements.

Primary Duties
  • Provides activities daily that expand and enhance the lifestyle of each resident in the seven categories required by the State and Federal regulations of creativity, physical, emotional, spiritual, group, individual, and social.
  • Develop, plan, and lead stimulating activities for residents as determined through resident preference, ability, and interest.
  • Maintains daily documentation for the activity department through resident records, progress
    notes, one-to-one sheets, participation records, and behavior charting.
  • Ensures that all activity department, facility, and division policies are maintained.
  • Responsible for offering a variety of activity choices, maintaining scheduled activities as planned, and facilitating spontaneous activities that meet the psycho-social, spiritual, emotional, physical, and creative needs of all residents.
  • Responsible for preparing in advance of the scheduled event, setting up necessary supplies, communicating to residents the activity is about to begin, assisting residents to/from
    the activity, encouraging residents throughout the activity, and maintaining positive atmosphere
    that enhances participation. Seeks guidance for difficult residents, supplies that are not readily available, and any other special needs.
Other Duties as Assigned
  • Answer call lights as needed, assisting residents with non-clinical needs (close the blinds, adjust the TV, etc.)
  • Other duties as assigned.

Minimum Qualifications


  • Must have a high school diploma or General Educational Diploma (GED)
Substitutions
  • Current, valid certification as a Nurse Aide (CNA) from the Colorado Department of
    Regulatory Agencies will substitute for the high school diploma or GED
Preferred Qualifications
  • Current active CNA License
  • Experience working in a long-term care or assisted living facility
  • Experience working with the elderly population
Conditions of Employment:
  • Must submit to and pass a full background to include, CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks (Skill Survey), Drug Screen, CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers)
  • Must have a current, active NA license from the Colorado Department of Regulatory Agencies or other states that participate in the eNLC.
  • This position is "Essential Service Designation": you will be required to report without delay or interruption to provide essential or emergency services to ensure health, safety, and welfare.
  • Must pass a TB test.
  • This position has shift work: must be available to work day, evening, and night shifts including holidays and weekends
  • On-call Status – If needed, per Pay Plan
  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination, or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
