Position Overview:

We are looking for motivated and compassionate Service Providers to join our team. In this community-based role, you will have the opportunity to manage your own caseload, set your schedule, and work in the community. Being a Service Provider encompasses many possible opportunities, including but not limited to:

 Participation in Case planning

 Supervised visitation

 Job Readiness

 Coordination of services

 Facilitate transportation

 Facilitate support groups

 Participation in Child and Family Team (CFTM) meetings

 Family reunification/preservation

 Community referrals and follow-up

 Develop structure/time management

 Behavior modification

 Budgeting/money management

 Meal planning/preparation

 Parent training with children present

 Monitor Progress of Parenting Skills

 Community Services Information

 Develop Long and Short-Term Goals

 Tutoring

 Father Engagement Services

 Truancy Termination Classes

 Mentoring

 Independent Living Skills

 Participation in Court Hearings

 Document client Progress Notes and Reports in Case Management Pro (CMP)

 Consistent Communication with Referring Sources

Responsibilities:

 Conduct comprehensive assessments of client's needs and develop individualized care plans.

 Collaborate with other healthcare professionals and community resources to coordinate

services for clients.

 Maintain accurate and up-to-date client records and case notes.

 Monitor client progress and make adjustments to care plans as needed.

 Advocate for client's rights and access to necessary services.

 Participate in occasional team meetings and case reviews.

 Stay informed about changes in relevant laws, regulations, and best practices in case

management.

 Maintain client confidentiality and adhere to ethical standards.

Qualifications:

 Associate’s or Bachelor's degree preferred in social work, psychology, or a related field is preferred, but not required.

 Experience in case management or a related field appreciated but not required

 Excellent communication and interpersonal skills.

 Strong organizational and time management abilities.

 Ability to work independently and effectively in a community-based environment.

 Empathy, compassion, and a genuine desire to help others.

 Proficiency in using remote communication and case management tools.

 A valid driver's license and insurance with no more than 6 points on driving record

 Background Check

 Travel between appointments to service clients within their community

 Knowledge regarding social service systems within the state

 Knowledge of and adherence to current confidentiality practices

 Appreciation for and ability to respond to cultural differences

 Develop relevant networks of contacts

Job Types: Full-time, Part-time, Contract

Salary: $20.00 - $25.00 per hour (Rate per hour is determined by the company services are contracted with)

Schedule:

Work with families to determine schedule that accommodates their availability and yours

Weekends may be needed depending on client’s needs

License/Certification:

Valid Driver's License (Required)

Willingness to travel:

100% (Preferred)
