**Job Summary:** Neighborhood Alliance, a longstanding community-focused organization, is seeking a dedicated Case Manager to join our team. As a Case Manager, you will play a crucial role in supporting individuals and families in need by providing comprehensive case management services. Your responsibilities will include coordinating and advocating for necessary resources to empower our clients to lead fulfilling lives. **Responsibilities:** - Conduct outreach and engage with individuals in various community settings to assess their needs and eligibility for services. - Develop and implement individualized service plans to address clients' housing, mental health, and other support needs. - Provide information, referrals, and linkages to community resources such as mental health services, addiction counseling, childcare, and workforce development programs. - Monitor and follow up on clients' progress to ensure they are connected to and benefit from the recommended services. - Assist clients in accessing public benefits, including Social Security Disability, and support them through the application process. - Respond promptly to emergency situations, including mental health crises or weather-related challenges. - Maintain accurate and timely case records using designated forms and systems. - Collaborate with other Neighborhood Alliance programs to provide holistic support to participants. - Attend program meetings and training sessions as required. - Perform any other duties assigned by the supervisor to meet the organization's objectives. **Qualifications:** - High School Diploma or GED Equivalent required; Bachelor's degree in Social Work or related field preferred. - Proven experience in case management or social work; familiarity with behavioral health, child welfare, crisis intervention, or addiction counseling is advantageous. - Strong communication skills with the ability to effectively interact with diverse populations. - Proficiency in data entry and Microsoft Office applications; experience with case management software is a plus. - Ability to work collaboratively in a team environment while also being self-motivated and capable of working independently. - Compassionate demeanor with the capacity to remain calm and think clearly during challenging situations. Job Type: Full-time

Pay: $16.00 - $18.00 per hour

Experience:

  • Case management: 1 year (Required)

Language:

  • Spanish (Preferred)

License/Certification:

  • Social Work License (Preferred)

Work Location: On the road
